The waters of Lake Malawi contain bilharzia, that dreaded worm which burrows into the skin, grows in the liver, and causes irreparable damage. To combat the parasite, pharmacies along the lake sell the drug that kills bilharzia for a low price and without prescription (back home a prescription is required). The lake water itself is clear, warm, and irresistible. So long as we swim in open water, away from reeds, the chances are low that we'll catch bilharzia.
